Acknowledgement of Attendance Expectations: My child and I understand the importance of respectful behavior and regular class attendance. We also understand that he/she may be dismissed from the program after two unexcused tardies or absences, or three documented incidents of poor behavior. Notification of a problem will be made to me by phone/mail after the second incident.*

Acknowledgement and agreement of Starting Arts Waiver: I hereby release, waive, discharge, indemnify, and hold harmless Starting Arts, its directors, officers, employees and agents, from and against any claim for damage, injury, loss or death to the registered student resulting from participation in any class, program, production or other activity either at Starting Arts or at another location. *

I acknowledge that each family is required to complete 4 mandatory volunteer hours, and that any unmet hours will incur an $80 buyout.*

I acknowledge that my child must be present for all Tech Week rehearsals, including both performance days, during the week of 4/6–4/10.*
